Vet Consent and Referral Forms
In accordance with the Veterinary Surgeons Act 1966, it is a legal requirement for all Veterinary Physiotherapists to obtain veterinary consent prior to treatment of any animal. Your vet does not have to see your animal before your veterinary physiotherapy appointment, but they must consent to referral.
Please download a veterinary referral form from the links below (either word or PDF) and then -
1. Fill out the owner sections of the form.
2. Send the form to me and I can send the form to your vet on your behalf, or send it directly to your vet and ask them to send a completed copy to me.
Alternatively you can contact Sophie to directly contact your veterinary surgeon on your behalf.
For performance or maintenance care in sound, healthy animals without any previous clinical conditions, vet consent may not be required. However, your animal must be registered with a veterinary practice and Sophie may then contact your vet to notify them of the intention to see the animal for maintenance purposes.
